2. Release of Liability: I agree not to hold Kids Goal Soccer Academy, its staff, volunteers, or coaches responsible for any injuries or damages that may occur during my child’s participation.
3. Indemnification: I agree to cover any costs or damages that may result from my child’s actions during the program.

4. Emergency Treatment: In case of a medical emergency, I authorize Kids Goal Soccer Academy to seek necessary medical treatment for my child. I understand that I will be responsible for any related medical expenses.
5. No Insurance Provided: I understand that Kids Goal Soccer Academy does not provide medical or other insurance for participants.
